Aug 2, 2023 · Usual Pediatric Dose for Peritonitis. Neonates (0 to 4 weeks): 30 mg/kg via IV injection over 3 to 5 minutes every 12 hours. Infants and children (1 month to 12 years): 30 to 50 mg/kg via IV injection over 3 to 5 minutes every 8 hours. Maximum dose: 6 grams/day.
