The salt-and-pepper appearance is characterized by the presence of vitiligo-like depigmentation with perifollicular pigmentary retention. The rich capillary network surrounding the hair follicle preserves melanogenesis, and hence the retained perifollicular pigmentation in systemic sclerosis.[ 2 ]

The salt and pepper appearance, first described in 1987 by Olsen et al, is a characteristic imaging feature of paragangliomas, found on T1 and T2 MRI sequences, reflecting its hypervascular nature (See Fig. 1 and Fig. 2).
