Reaper gains 20% of damage dealt to other heroes as health. So if he shoots someone for 100 damage, he regenerates 20 health for himself. This applies to his weapon damage, melee, and ult and only works on enemies (so shooting into shields or turrets, for example, will not regenerate your HP). Reply.
This is not about Reaper's ult, this is him doing regular damage. But about ults: A few (Hanzo and D.va come to mind) can gain ult charge while their ult is in effect, none gain ult charge from the ult itself.

I think reapers self healing right now is very strong and not really balanced, I preferred him healing after each kill, cause his primary fire is easy to aim and kind of fast. That makes him heal really fast basically making the enemies shots unnoticeable.
